Emergency Health Cards for Individuals with Disabilities

Guam Legal Services Corporation-Disability Law Center will be providing Emergency Health Cards to individuals with disabilities. These health cards will contain information that will be helpful for medical workers or safety officers to assist individuals with disabilities during an emergency crisis.

These cards will be provided by GLSC-DLC at no cost to individuals with disabilities.

*NOTE: These emergency health cards are for emergency assistance only and are not official identification cards.

Once you are done filling out the application, save the form, and attach it to an email together with your photo ID. Please email your application form and photo ID to information@guamlsc.org.
